Hmmm, mine was deposited on February 1 in my USA bank account.

Update: Emailed the US Embassy yesterday morning, Mon. the 4th. at 8:30 am and explained the situation. Just gave them my full name. 8:47 they emailed me back wanting DOB, Place of birth, last 4 of my ssn and a few other things. Sent the info they needed at 8:55 and they replied back at 9:04 that SSA had not received the dead or alive form from me. They included the form I needed in a self fillable PDF attachment that I filled out right away..put it on a usb and asked my GF to go to the print shop for a copy. Signed the form,took it to LBC and mailed it to the Embassy. Done by 10:15 . The embassy is closed today for the Chinese new year so I'm thinking maybe tomorrow they can enter it into the computer. Could have faxed it but don't know anyplace around here that has a fax machine.

Those employees at the embassy are really good and efficient. I'll update again when The money is deposited in my bank. (US Credit Union)

From what I have read, the Embassy just forwards the form via pouch to the appropriate SS department.  So it will take a bit longer to get to them in the U.S

Let us know if I am wrong about that.  It would be good to know that the embassy can take care of it directly.

I have been searching for information on the SSA "proof of life" process and I am amazed that I can find nothing on the SSA main website.  It seems like it would be important enough to have some reference information there.

However, I did find some better information on some U.S. embassy websites.  The Costa Rica site has an updated notice:

Just keep in mind that some of the information is only for the countries in their jurisdiction.  Don't fax to that number or e-mail them.

I'm going to make a tickler for next December 1.  If I have not received the form, I will download it and send it by Fedex or DHL to the Wilkes Barre address.  For my kids too.

I did receive my form(proof of life) from SSA in Jun. Took about 3 weeks by standard(slow) mail to the local PI post office. Filled out the form and noticed it was to return to PO Box. So I used LBC to send to my son in Calif. Instructed him to put a stamp on it to send SSA Po box. Works okay, but you need someone in the States to send it to and put a stamp on it and forward from there.

Good to know.  I had read that they were mailed around June and I was going to ask for confirmations.

So, if not received by say, September, consider it lost, then download the form and mail it.

Just out of interest, why not download it anyway in June and save a few dollars on express postage?

For important documents that I don't need to send by express, I send 2 copies on the basis 2 getting lost is unlikely.
