So maybe a silly question. Say you want to stay for 3 months. Can your return flight be booked after your initial 30 day Visa date has expired? The intention would be to extend at the airport on arrival or at an office during the 30 day Visa for extension.

No. Return ticket must be within the first 30 days. Airline often ask to see return ticket.

Immigration always ask.

Search rent a ticket,  or buy a throw-away cheapest ticket, one way flight Manila to Malaysia (used to be down to 20 USD) Now?

Your main ticket can still have retutn date say 90 days later, or whatever you like. Just show the earliest return ticket at check-in, and to Immigration
