I have seen it done so many times that I assumed it would be in there somewhere.  You know what they say about "assume".

Mea culpa.

But I do know that is how it is done.  If not in the law then in the culture.

That is also how my wife's family is named.  This is Moalboal, Cebu.

Similar here.  My partner is from Oriental Mindoro and when I asked her surname it is the same as her first name.  She explained it's her Mother's maiden name and everyone uses the Mother's maiden name as a middle name where she comes from. Her Mother happened to marry someone with the same surname so everyone in the family has the same middle name and surname.

My (Phils-born) 5 year-old son has his UK-style first and middle names, followed by his mother's maiden name, then finally, my surname. 

He has both Filipino and British passports.
